Mahitahi Hauora is leading a bold new programme to improve care for people with long-term conditions in Tai Tokerau.
Long-term conditions are a significant contributor towards morbidity and mortality in Northland, especially among Māori, who are at greater risk – something the existing services are failing to change.
The Long-Term Conditions Programme targets inequity and high-priority areas, using a data-driven approach. Referral criteria to services ensure priority groups such as Māori, Pacific peoples, and high-deprivation populations are targeted to receive services.
The Programme encompasses three service delivery projects, which will be developed in 2023 using quality improvement methodology. The three projects are the Spirometry Coordination Service, Gout Stop, and Prediabetes:
Spirometry Coordination Service – The purpose of this project is to stand up a Spirometry Coordination Service to increase access to spirometry across the rohe.

Gout Stop – The Gout Stop project is reinvigorating and improving the existing Gout Stop service, which supports general practices and pharmacies to treat and manage gout. The project aims to increase the number of Māori who successfully complete Gout Stop at 12 months by 30% by June 2023.

Prediabetes – The Prediabetes project is developing a pilot programme for patients with a new diagnosis of prediabetes and overweight. We’re partnering with Hauora Hokianga to design and test the pilot with input from the community.

Each project is centrally coordinated, with a kaiawhina, a registered nurse, resource development, a communications strategy, and workforce education.
