A NFP Charitable Trust, Mahitahi Hauora PHE works collaboratively with community, primary healthcare providers and key partners to support general practices, Māori health providers and social sector entities in achieving better health outcomes for our Tai Tokerau population.
As a primary healthcare organisation underpinned by Te Tiriti o Waitangi our Kaupapa includes ensuring whanau and communities are able to achieve self-determined wellbeing, access to the services they determine they need, and to live a long and healthy life.
What does the position do?
Working as part of a team, you will support a number of practices and providers across the greater Whangārei region. Your clinical knowledge will apply to the implementation of a variety of programmes in general practice aimed at achieving improvement in health outcomes, including immunisation, long term conditions, cervical screening to name a few. The primary healthcare environment is continually changing to meet the needs of individuals and whānau, and this role is critical in supporting efficient and smooth practice function.
You will have interaction with general practice teams and work closely with them supporting improved models of care aimed at increasing population access to services and better patient outcomes. Your knowledge of clinical models and pathways will be applied to facilitate change management associated with quality and system improvement.
As an RN you will work with the wider team to support workforce capability and development in managing new programme roll outs and working to improve referral pathways, MDT’s, and access to services that will ultimately improve health outcomes for our Tai Tokerau enrolled population.

You will be a back-up for the GP Services Leader in their absence, and support the continued work of the team during these times.

Ideally you will be a relationship driven person, passionate about improvement in patient outcomes, and enthusiastic in your approach. You must be prepared to be flexible and innovative in your work.
Having a good understanding of the primary healthcare and community environment would support easy integration into the role, however to support your success in this role, you will undertake broad orientation and training.
