Partnership Services Team

Our Partnership Services team makes it easy for frontline primary health care providers to deliver their mahi. We work directly with general practices to provide tailored services to help meet the needs of enrolled populations. We also coordinate referrals to support whānau, and support access to interventions to enhance health and wellbeing.
