Physiotherapist – Up to 32 Hours per week

This role will provide lots of variety working across and within multiple practices as part of an interdisciplinary team.

 This exciting opportunity has arisen where we are able to offer a part time flexible hours position for an experienced Physiotherapist who will support a new national initiative “Comprehensive Primary Community Teams” (CPCT) in primary healthcare for Whangarei and the Mid North.  This is a great role with plenty of variety in working with multiple practices.

This new initiative aims to support an increase in access to services, and equitable health outcomes through integrating a range of roles including Pharmacists, Physiotherapists, Care Coordinators and Extended Care Paramedics, into existing general practices.  This model along with improving health outcomes has a strong focus on expanding the skill mix available to primary healthcare teams and includes support for specialist pathways development for each of the roles. With a priority focus on Māori, Pacific and rural people, these teams are intended to help reach more of our at-risk communities and reduce barriers to accessing care.

About Mahitahi Hauora

Mahitahi Hauora is a primary health entity underpinned by Te Tiriti o Waitangi, our Kaupapa includes ensuring whanau and communities are able to achieve self-determined wellbeing, access to the services they determine they need, and to live a long and healthy life.  As the largest PHO in the Tai Tokerau region, we work collaboratively with community, primary healthcare providers and key partners to support general practices, Māori health providers and social sector agencies, in achieving better health outcomes for our Tai Tokerau population.

The Position – Physiotherapist

The primary purpose of the role is to provide physiotherapy assessment and care interventions across the comprehensive primary and community team for people and whānau at greatest risk of poor health outcomes and to actively support hospital avoidance through coordination of care for early supported discharge and hospital avoidance activities.

As an integrated member of the CPCT, and the General Practice interdisciplinary team (IDT) you will be actively involved in meetings and using the systems and processes that support IDT functioning and outcomes.   A key purpose of this role is to support increasing the capacity for other members within the general practice interdisciplinary team.

 Your experience and background should be able to demonstrate the following:

  • Assessment, diagnosis, and treatment of acute and chronic conditions within scope of practice.
  • Provision of individualised exercise or rehabilitation programmes, including manual therapy, self-management support and education for areas of health, including but not limited to musculoskeletal and other long-term conditions.
  • Provision of group exercise, rehabilitation, or self-management programmes.
  • Prioritisation of people and whānau to support hospital avoidance, early supported discharge from hospital and those at greatest risk of poor health outcomes.
  • Clear strategies to simplify care for whānau where able, promotion of self-management approaches and intensifying interventions for those with the most complex needs.

Successful applicants will need to meet the following criteria however, we are interested in hearing from Physiotherapists with a range of experience levels and backgrounds particularly as support for specialist pathway development may be an option for this role.

  • Bachelor of Physiotherapy or equivalent overseas qualification
  • Current Annual practicing Certificate (APC) with no restrictions (those undertaking prescribing activities must be registered in the Physiotherapist Prescriber Scope of Practice)
  • Current Registration with Physiotherapy Board NZ
  • Comprehensive understanding of inequities in access and health outcomes and a detailed knowledge of Te Tiriti o Waitangi and its application to shaping all efforts within primary care
  • Experience working in the Primary and Community healthcare environment
  • Advanced written and verbal communication skills
  • Excellent time management and organisational skills
  • Excellent critical appraisal skills with the ability to identify the best evidence-informed solutions to clinical and practice questions and issues

You must be a NZ Citizen or Permanent resident or have a current open NZ work visa.
