Primary Care Development Coordinator 1.0FTE Whangarei

Full time permanent position

This role has lots of variety.  As the Primary Care Development Coordinator, you will be able to utilise your professional nursing skills and knowledge to support primary care in implementing and responding to changes and opportunities emerging as part of the health reforms.

If you are a Registered Nurse who is equity focussed, enjoys working with a range of stakeholders and who is comfortable and capable of providing professional advice and education to support the growth of capability in primary healthcare environments, this role could be just the challenge you need.  Working closely with other teams, you will support primary healthcare to improve patient and whānau health outcomes and access to services.

Mahitahi Hauora is a primary health entity.  Underpinned by Te Tiriti o Waitangi, our Kaupapa includes ensuring whānau and communities have access to the services they determine they need, are able to achieve self-determined wellbeing,  and to live a long and healthy life.  We work collaboratively with community, primary healthcare providers and key partners to support general practices, Māori health providers and social sector agencies, in achieving better health outcomes for our Tai Tokerau population.

The Position

As a Primary Care Development Coordinator your role will be working across multiple areas at once, including providing advanced/specialised clinical leadership and advice, supporting workforce capability and development in managing new programme roll outs and working to improve referral pathways and access to services that will ultimately improve health outcomes for our Tai Tokerau enrolled population.

You will need to have a broad understanding of the primary healthcare sector and the integration of services between primary and secondary care.  You should also have a general understanding of the current challenges faced by communities accessing first level health care services.

To be successful in this role, you will need to be experienced in implementing programmes and projects and enthusiastic in supporting and addressing the needs of our primary healthcare environment.

To be considered for this position you must:

  • Be a Registered Nurse with a minimum of 3-5 years post graduate experience
  • Have post graduate studies in specialty or related field
  • Have proficient level or above on PDRP
  • Have an understanding and preferrable experience in primary/community healthcare
  • Have experience in facilitation, change management coaching and educating
  • Be experienced in developing and implementing services and/or programmes
  • Have advanced computer literacy across MS Office 365 suite
  • Demonstrable experience in developing and managing relationships with healthcare professionals
  • Be a confident communicator both spoken and written
  • Must have NZ residency or citizenship, or hold a current open NZ work visa.
