About Mahitahi Hauora

Mahitahi Hauora is a primary health entity.  Underpinned by Te Tiriti o Waitangi, our Kaupapa includes ensuring whānau and communities have access to the services they determine they need, are able to achieve self-determined wellbeing, and to live a long and healthy life.  We work collaboratively with community, primary healthcare providers and key partners to support general practices, Māori health providers and social sector agencies, in achieving better health outcomes for our Tai Tokerau population.

The Position

This position is based in Whangārei and supports a number of practices and providers across the greater Whangarei region.  Primarily alongside your Practice Facilitator colleagues you will support practices and providers across primary healthcare programmes, improvement methodologies and equity strategies.  As an RN you will also support workforce capability and development in managing new programme roll outs and working to improve referral pathways, MDT’s, and access to services that will ultimately improve health outcomes for our Tai Tokerau enrolled population.

You will need to be a relationship driven person, passionate and enthusiastic in your approach, and be prepared to be flexible and innovative in your work.

Having a good understanding of the primary healthcare and community environment, and the challenges faced in accessing first level health care services is important.

To be considered for this position you must:

  • Be a Registered Nurse with a minimum of 3-5 years plus post graduate experience
  • Have proficient level or above on PDRP or demonstrable experience
  • Have an understanding and preferrable experience in primary/community healthcare
  • Have experience in facilitation, change management and educating
  • Be experienced in supporting and implementing services and/or programmes
  • Have advanced computer literacy across MS Office 365 suite
  • Demonstrate ability to develop and manage relationships with healthcare professionals
  • Be a confident communicator both spoken and written
  • Must have NZ residency or citizenship or hold a current open NZ work visa.
